("Applicant") located at 151 Christian Hill Road, Great Barrington, MA 01230 intends to file an "Application" with the Massachusetts Department of Public Health for a Conservation Long Term Care Project for Fairview Commons Nursing & Rehabilitation Center, a 146-bed skilled nursing facility, located at 151 Christian Hill Road, Great Barrington, MA 01230 (the "Facility") to renovate the existing Facility. These renovations include roof replacement and storm drainage, repair of roof rafters, cinder blocks, and efface, exterior painting, HVAC replacement, driveway paving, sidewalk replacement, plumbing and electrical upgrades, dietary flooring upgrades, and renovations to resident rooms and common areas (the "Proposed Project"). The Applicant does not anticipate any price or service impacts on the Applicant's existing Patient panel as a result of the Proposed Project. The Total Value of the Proposed Project based on the Maximum Capital Expenditure is $5,575,000.
